online intake forms. it does not make any sense for families to have to download and upload an intake form to fill it out. I saw someone else asked for this back in 2020! PDF is not a current system for intake forms
We would love to see the intake forms have an electronic submission option.
Currently, we continue to use another company for our intake forms because it allows the electronic submission. Most of our patients complete their forms on their phones and push "submit" while "on the go" in their daily routine. We would love to maximize Fusion's intake forms and allow it to submit electronically and show in the patient's portal.

This is such a challenge for our patients and adds to the work of our front office team. Patients should be able to enter the info through the portal and have it automatically enter in Fusion. Printing and scanning is inefficient and is wasting so much paper. Please fix this! Thank you!
YES - this is SO difficult for less tech-savvy patients - we are always hearing back from people who are having trouble, end up printing the forms, filing them out and bringing the PAPER to us for scanning and uploading. Fusion needs to fix this - put it at the TOP of your to-do list, software engineers! Thank you!
Please, please, please get fillable patient forms in the portal on the fast track. The patient should be able to enter information electronically and it be automatically entered into a patient chart with provider approval.
Fusion NEEDS to allow online PDF printable forms to be completed through the portal withhold having to download and scan back. This function would save much time for the patients and also the staff!
Our families are having a difficult time completing patient paperwork from the portal. Many of our families do not have computers. It would be very helpful if they could fill out the forms on their phones. Is there a way to make them fillable from the portal rather than having to download and upload back to portal?
PLEASE come up with a way for patients to fill out the forms without having to download them or print them! The process now does not work and we end up spending too much time having patients complete the paperwork in the office when they come in, sometimes even pushing into their appointment timeslot.
Intake forms on the portal easier for families to review, fill out, and sign! I get a lot of complaints that they have to download, fill out, then re-upload them to their portal. It would be wonderful if Fusion had their own system so we could upload our forms but it was all completed through Fusion.
We love fusion, but we really wish there was a way to send our intakes via fusion without the clients having to download and re-upload the forms. In other EMR's, the companies have charged us additional $$ for their reps to be able to spend the time and create personalized forms that were meant for our company- I guess fusion could have their own templates, too. This would be SO helpful.
How about online, tablet/iPad compatible intake forms? Anything in the works for this? It's too much to ask our patients to fill out forms and reupload them to their portal! Something that automatically fills in the intake part of Fusion would be AMAZING!
Please, please, please make the intake forms on the patient portal more user friendly. Can you simply allow the editable forms to be edited within the portal without having the user download, save, and re-upload? Or, better yet, can you make the intake forms able to be completed and customized in system where information is automatically inserted into a patient's chart?
Please fix the patient portal so its easier for the patients to submit their paperwork or fix it so its a fillable document instead of having them upload it to the portal. The documents are blank when patients upload them.
