Send and receive intake forms with the patient portal

This is a great START, however this is not what I envisioned with Fusion intake forms. This process suggested in the video will likely take as much time as we currently need to input and transfer all information manually. This is not what a "paperless" clinic should be doing. When submitting my suggestion, I thought it more ideal for the intake form to be EMBEDDED/with the capability of the parent to complete online, so that all patient information automatically loads into the patient file in Fusion, in addition, parents/caregivers should be able to securely digitally sign all intake form documents. It currently takes double the time and effort for our office to have to do it manually, and in addition, this does not make our clinic "paperless" - which we believe is our ultimate goal and Fusion being an EMR system should provide. In conclusion, will Fusion move forward to create an EMBEDDED intake form option, one in which will keep all information seamless in Fusion? The suggested added steps to download, print, fill out and manually scan and upload back into Fusion is actually added time for the patient and double the work for our office/clinic. This does not promote office efficiency at all, in fact, it makes us less efficient. While the efforts are appreciated, we'd greatly appreciate you considering the above mentioned addition to the original suggestion and add the ability for embedded intake forms in which all patient information entered by the patient can be seamlessly uploaded into the patient file. Will Fusion be adding an "intake" component to its Platform? It would be helpful to be able to have a client intake/booking appointments feature.

PLEASE add intake forms to the parent portal. It would be so amazing to be able to have our intake process streamlined with Fusion. Right now we have them fill out paper or a google form, and then have to scan and upload into Fusion, and then have to fill in all of the contact information. It's cumbersome and makes the process so much less efficient.

Fusion had previously requested feedback on what would be helpful to add to Fusion Web Clinic. I know Fusion is working on sending mass emails. One feature that would be helpful to add is patient forms that can be sent from Fusion (like e-doc forms). Not sure if that is a possibility?

Add feature for HIPPA compliant intake forms.

There should be an electronic option to have the patient's sign the consent to treat and hipaa notice electronically and save it electronically.

patient portal needs to be a place patient enters all referral forms with patient hx. This needs to upload straight to evaluation

It would be nice to have the intake paperwork and questionnaires available online through the patient portal to where they could have it completed before their visit. Even allow them to upload photos of insurance cards. It would also be nice to be able to send documents that need a response from the parent. I would like to have a feature where they could send us (admin staff or their therapist) a message through their portal if they have questions about something.

Can we get an intake form and policies/procedures/waiver, etc. built in to the system? One that we can email from Fusion to parents that will fill in designated fields once completed.

Perhaps there would be a way we could either send our initial paper work documents to patients for completion from the fusion platform...or have a way that they could complete it from the patient portal?

It would be great if Intake forms could be sent from Fusion. The family could complete online and the information could automatically be inserted into the patient information and/or evaluation as appropriate. Any consent forms could be saved within attachments in the patient information section. Thanks for being open to suggestions!

We would like to have a way to send patients our new patient form to fill out through fusion so that we do not have to manually enter it each time.
