Make "Yes" the default option for "Include graphs" on progress notes and reassessments.

There needs to be a way to include the goal data by default on progress notes and reassessments. Or an option in the admin side to make those goal graphs by included by default on certain documents and let you choose. It would be nice to have an option to include it on daily notes too.

Default for graphs always show up on progress notes from last assessment.
