Make Kiosk check-in easier

Fix date of birth on the kiosk to a more user friendly format, such as automatic ___/___/____ format

YES! So frustrating to explain to each client over and over how to either use a backlash in the first place, or that the calendar wont work because its set to 2019 and they'd have to go back YEARS to get the right date, or that the hyphens wont work. Elderly clients are especially not into signing in because of the non-intuitive format.

Add the backslashes (/) in the birth date section in the kiosk because some parents have difficulty singing in without those present.
