Display patient's first name, last name and nickname on profile

When viewing a patient's profile, I want to see the patient's first name, last name and nickname in the top left.

    When you open up a patient's information/file the full name should show in the top pane with the nickname in quotation marks. Currently only the nickname and last name shows up.
