Include a communication log in the patient's information

Add a Communication Log to the patient's information screen so we can have a separate place to just note communication with the patient/caregiver. If we need to go back and reference anything it will be easier to find because all of the communication will be in one place.

      I'd love to have an option to create a contact note to document phone conversations/contact with family and other providers that occur outside of appointments.

      I want a designated place to document phone and email conversations between the therapist and the client/parent. These conversations sometimes occur between appointments (so aren't included on daily notes). They get lost in the case notes section, which is also able to be edited by anyone.
