There needs to be an option on the patient information tab for each payer that you can select yes or no if a plan requires auth and then the claim will mark any claims as a problem if the patient doesn't have auth if their box is checked yes for requiring an auth. This is mainly a problem for us with Tricare and United Healthcare. Some of their plans require auth and some don't. We cant use a billing rule because it doesn't apply to all of their patients.
