Report on completed documents

Allow me to run an insight report on documents that have been completed within a specific timeframe that includes the primary therapist and co-signer. We send all assessments to doctors and want to just be able to go through a list of documents that need to be sent.

      Add an option to include the name of the supervising therapist to the appointments report so that I can run a report to see who the supervising therapist that signed off on documentation is.
