    Send out patient survey to patients being discharged through Fusion.

    Surveys by email or text to send to patients after initial evaluation, intermittently during treatment, and after discharge.

    We love utilizing the parent portal and the access it gives families! It would be really great if we could create a simple "checklist" or questionnaire - ex: "Have you noticed improvement in (blank) area?, Do you have questions about (blank), has anything changed in the last (blank)" Whenever parents log in, perhaps periodically, the portal prompts them to answer these questions. Results are aggregated to a report or as a notification to the clinic.

    Survey feature for parents. 1. Scheduled automatic surveys that go out on a scheduled based on the date of the evaluation. Ex. A survey 1 week after the eval asking how the eval was, A survey 3 months after eval asking how therapy is going, then a reoccurring survey every 6 months until discharge. 2. Mass surveys based on if patients are active/discharged. 3. Individual custom surveys. For example if grandma brings the child to therapy, send a survey to the parent asking about how the child is doing with a particular problem. 4. The ability to attach survey results to a note (daily/progress/eval). 5. Process: admin sets up mass ro scheduled emails. Clinicians go to EMR chart or note documentation to create/send the survey. Parents get an email notification and complete the survey in the parent portal. Survey results show up in the EMR chart and attached to note if connected with a note.

    Hello! Do you have a way to send follow-up texts with a simple survey such as "how was your recent visit"? With the addition a few short questions. Thank you!
