Merge invoices

Let me merge multiple invoices together into one so there's a single invoice with all of those lines.

    It would be nice to be able to combine individual invoices to make it easier for patients to make one larger online payment instead of multiple individual payments. Also the option to set up payment plans on patients accounts. To either run a card automatically at a set interval or send a reminder to manually run a saved card.
