Allow clinic to use cards saved on the portal

Allow caregivers a way to grant the clinic access to the cards that they have saved on the patient portal and permission for the clinic to use that card for future transactions.

Add "mastered" in the goal comments of the daily note

If the goal has been mastered, add the word "Mastered" to the comment field for that goal in the daily note. It would be easier to view it there rather than underneath the goal.
