Add separate entries and timestamps to note fields, like "Coverage Notes"
I want to keep track of each note a user adds and when it was added, so multiple notes can be maintained with timestamps. For example, a patient's "Primary Coverage Notes" field could allow for multiple notes instead.

I want to set up different templates for documentation based on the type of service that pre-selects content, like test & measures or performance skills.
Allow me to set the co-pay amount based on the service type. Right now there is only one co-pay field per payer and we have payers that set different co-pay amounts based on the type of therapy.
