Simplify Billing Codes in Documentation

Make it easier for me to add and edit billing codes in documentation. Right now there are too many dialogs that open.

    Billings codes are now added and edited inline.

    Entering charges (incomplete note) - Add the submit charges button on the bottom of the charges page. If a user is just entering charges, there seems like too many steps to enter them, save them, click on submit, then verify.
