Add baseline for goals

Add a field to the goals for baseline data which also appears as a separate dotted line on the goals graph. (TRICARE is specifically requesting this information for ABA patients.)

Has any other provider requested that "baseline status" be added under the long and short term goals. NC Medicaid and insurance requires this information on our goal plan and we have to add it each time and would be helpful if there was a tab for it within the long and short term goals.

Make a place for baselines for goals

Create a field to enter baselines of data for short-term therapy goals.

Display goal starting data along side of the current data so the goal baseline is always visible.
