Add chronological age to all documentation

Add the patient's chronological age to the top of every completed document.

    In the patient header there should also be a field that gives the child's age or have the age in parenthesis after the date of birth

    It would be wonderful if, in addition to the DOB, the Evaluations, Progress Reports, and Discharge reports would also include the child's chronological age in the headings. thanks!

    have pt chronological age in demographic section of evaluations. (notes etc)

    Please include Chronological Age on documentation, not just on the patient screen but on the document itself. This is an expectation in the therapy field and is a glaring omission on Fusion's part.
