Add "Other" Option to Referred By

In the patient information screen, allow me to choose an "other" option for who referred the patient.

    The "Referred By" field is now styled in a way to indicate that you can type whatever response you would like rather than being limited to the listed options.

      In the patient demographic area, under "Referred By" section, I would love either a "self" option, or an "other" option. I have referrals that don't come from any of the current options to select, and there are no other ways to indicate such. Thanks.
