Add notes to unavailable times

When a user's unavailable time is added, allow a place to write notes or comments so that if someone needs to know where they are, they have a way to contact them.

    Wish we could block out a whole week for vacation and have it show up on the top of the calendar across the whole day/week.

    If a therapist has an unavailable time greyed out on the schedule, add text so that I can see why the therapist is unavailable.
