Add additional therapist credentials

Allow me to add additional credentials. Some therapists are board certified in certain areas and we'd like that included with our signature. Also, some payers require additional statements after credentials before they will approve authorizations.

    We've increased the character limit when adding a user's credentials. Credentials can now contain up to 50 characters to accommodate longer or additional credentials.
