As a therapist, if I sign off on a document but my supervisor declines it, only the final sign date (the date it was accepted by my co-signer) is displayed on the document.
Display both sign dates on the final document, the date I originally signed the document and the date that it was accepted by my co-signer. Some payers require documents to be signed by a certain date.
