Remove copay field for private pay patients

For patients that we have set-up as private pay, don't allow me to enter a co-pay amount at check out.

    Copay's vs. Account Payments - It is so easy to accidentally put a payment under "Copay" when checking in clients. If a client is SELF PAY and an insurance isn't on file, I think the system shouldn't allow us to be able to put a payment in the COPAY field.
