Add physician information to patients report

Include a column for the physician's information on the patients report so I can see how many active and referral patients we have for each physician.

    I would love to see the physician added to the patient report on insight. I can get the patients name, therapy, dx code, payer, location but no physician. If there is a report out there that has all this info, please email me. Thanks Shelly
