Add column headers to every page of statements/invoices

When printing a statement or invoice that has multiple pages, have the column headers (i.e. Charge, Credit, Total, Balance) appear on every page. Since the totals are displayed on the last page it would be helpful to have the column headers to reference what each total is for.
