Link patients to a physician practice

Allow me to link a patient to a physician practice rather than a specific physician. We have some patients that don't really have a primary, rather they see whoever is available at a specific practice.

    it would be nice to be able to add a physician to a practice instead of having to add all the information in for the same practice.
