At the top of the document next to "Location", include the clinic's physical address.

When adding a goal, make the data type and target fields optional. We don't use the goal graphs in Fusion and it would save us time if we didn't have to enter this information for each goal.

Add the patient's phone number and email address to the evaluation document. If the patient gets referred out, having the phone number and email address would help our clinic and physicians in communication with those patients.
