Group evaluation heading on the same page as the data
If I'm completing an evaluation-type document and one of my sections runs over onto another page, have the heading for that section display on the second page as well. That way whoever is reviewing the document will know what they are viewing with...

When viewing reminders or my list of documents, make a greater distinction between each row. Sometimes I lose my place when I'm trying to read all the information in the row.

Add clinic fax & phone numbers to documentation footer
Add my clinic's fax and phone number to the footer of documentation so that the doctor's office can find our contact information in the event that the coversheet is misplaced or discarded.

When completing a progress note or reassessment, allow me to pull forward the previous assessment data onto the current document and have it displayed side-by-side with the new assessment data for easy comparison.

Allow me to send evaluation content to another therapist for them to use in their documentation so that information that is the same across all reports (e.g. history) doesn't have to be re-typed.

Switch the location of the "sign" and "save" buttons on documentation. The "save" button is the preferred action and I keep hitting "save" when I mean to sign the document.
